If you follow a service-specific inquiry link, its service interest is included; otherwise it is recorded as “Not sure yet.” The message may include a service, Nevada location, and timing if you choose to describe them. Older inquiries may also include a separately entered location. Each submission includes a timestamp and a reference identifier.

A daily hashed connection identifier helps limit excessive submissions. Our hosting and security providers may process routine connection information, such as IP addresses, request details, and technical logs, to deliver and protect the website.

How we use information

We use inquiry information to understand your request, discuss scope and timing, respond to you, and maintain a record of our communications. Technical information supports website operation, security, and abuse prevention.

Access and sharing

Form submissions are stored in a private inquiry inbox with access restricted to the authorized SRC account. Providers supporting website hosting, storage, authentication, and communications may process information needed to provide those services. This website is hosted through ChatGPT Sites, with Cloudflare providing hosting and database infrastructure.

We do not sell contact-form information or use it for targeted advertising. Information may also be disclosed when required by law or reasonably necessary to protect legal rights or respond to misuse. Information shared for an investigative engagement is governed by that engagement and applicable requirements.

Cookies and website technology

No advertising pixels, analytics scripts, embedded social feeds, or third-party font requests have been added to this site. We have not added tools that track your activity across different websites. The hosting platform may use necessary cookies or similar technology for access, authentication, and security. Fonts are served with the website.

Phone and email links open your chosen services. External links, including the licensing-board website, lead to services with their own privacy practices.

Retention and security

Inquiries remain in our private inbox until removed by SRC; the contact form does not apply an automatic deletion schedule. We retain information needed to handle inquiries, maintain relevant communications, meet applicable obligations, or address disputes. You can request deletion as described below; some information may need to be retained for legal or business reasons.

The site uses HTTPS, restricted inbox access, input validation, and submission limits. Please keep your initial message general. Do not send medical records, financial account details, identification documents, passwords, or detailed case evidence through this form. Discuss an appropriate method for assignment materials with us first.

Your requests and choices

Include your inquiry reference if available and describe your request. We may need to verify your identity before acting on it.

Providing optional form fields is your choice. You can also contact us by phone rather than using the form.

Changes to this policy

We post updates on this page and change the effective date. If a material change affects the use of previously submitted inquiry information, we will provide notice through the website or, where appropriate, the contact information available to us.
